Before your first visit, please complete the Delphi Enhanced Primary Care Authorization Form and return it to us by mail or by dropping it off at our office. If preferred, record requests may also be faxed directly to 888-927-0461 or emailed to ca**@*******lc.com. This form allows us to request your previous medical records. We have made this form available online, you will also receive a physical copy in the Welcome Packet.
