Your Next Steps as a Patient

Once you have registered as a patient and your membership has been confirmed, you will receive a Welcome Packet in the mail.

Your first step as a new patient is to complete the form titled “Authorization to Release Medical Records”. Once completed, please submit a copy to your previous primary care office as well as Delphi. We recommend submitting your records request as soon as possible to avoid delays in our ability to deliver you primary care services.

In order for your doctor to deliver high-quality, personalized primary care services, we require a copy of your medical records.

Please note: Under federal regulations, healthcare facilities have up to 30 days to process and respond to medical record requests. While many offices respond sooner, this timeframe is standard and outside of our control.

Forms and medical records can be mailed, handed into the front desk or faxed to 888-927-0461. If you require additional copies of these forms, please reach directly out to the office.

Updating Your Insurance & Pharmacy

Please contact your insurance carrier to designate your Delphi Doctor as your Primary Care Provider (PCP), if required by your plan. Please verify that they have updated the proper contact information to match Delphi Enhanced Primary Care. This helps ensure referrals and claims are processed correctly.

We also ask that you call your preferred pharmacy to notify them of your new provider and practice information. This will allow your prescriptions to be transferred and managed through your Primary Care Physician at Delphi.

Your Patient Portal and How to Use It

We use Healow Health as our patient portal. Setting up your account is quick and simple, and it gives you secure access to your health information. 

Once your first appointment has been scheduled, you’ll receive an email from Orchard Medical Group / Hampstead Family Practice with a direct link to register your Healow account. You can access the Healow Patient Portal login by clicking here.

Membership Fee and Benefits

The access fee allows Delphi providers to maintain a smaller patient panel, enabling same-day or next-day appointments and extended time with each patient. This provides a more personalized and accessible care experience.

Benefits include same-day/next-day appointments, more one-on-one time with your provider, and a higher level of continuity and personalized care due to the smaller patient panel size.

Yes. We are more than happy to schedule a 15 minute visit for you to meet your doctor of interest and see if Delphi Enhanced Primary Care is a good fit for you!

Payments are accepted via debit and credit cards, including American Express, Discover, MasterCard, and Visa.

With recent changes in federal law, as of 01/01/2026, employees can use their Health Savings Account (HSA) to pay for primary care memberships, giving the ability to use pre-tax dollars for increased access to high-quality, affordable primary care whenever needed!

Yes, the first member registered under an account will be the full price, the second individual registered under the account will receive a 10% discount. Please see our membership page for further family discounts.

Insurance and Appointment Billing

We accept a range of health insurance plans from most major carriers as well as Medicare. The specific plans we accept may change, so please call us if you have any questions.

Just like a traditional office, Delphi bills your insurance for in-office and scheduled remote visits, and you are responsible for copayments, deductibles, or coinsurance based on your plan.

Delphi Enhanced Primary Care uses Patriot Pay through our billing partner, Orchard Medical Management. Patriot Pay provides patients with convenient and secure access to pay their medical bills directly from their mobile phone. Following your visit, you will receive a text and email with the details of your visit and secure link to make your payment.

By responding to the text sent by Patriot Pay “STOP”, you will be opted out of text and email notifications and enrolled into paper billing only.

Delphi Doctors are dedicated to delivering high-quality, personalized primary care that fits each patient’s needs. As part of that commitment, patients may contact them directly after regular business hours for urgent concerns that cannot safely wait until the next business day or that are especially time-sensitive or sensitive in nature. At the same time, we kindly ask patients to respect that physicians are people too, with families and responsibilities outside of the practice. Our providers prioritize healthy work-life balance so they can continue offering attentive, compassionate care to every patient.

In the event of a medical emergency, patients should call 911 immediately and proceed to the nearest emergency room rather than contacting your doctor first. Once the situation has stabilized, please follow up with your doctor so we can support your continued care.

Delphi Doctors do not admit patients to the hospital, hospital admissions will be managed by the hospital’s care team. Your Primary Care Physician’s role is to advocate for you to ensure you receive the highest level of care coordination all the way through to follow-up.

Delphi Doctors will periodically be out of office to attend conferences or on vacation. During scheduled absences, arrangements will be made for an alternate Delphi Doctor to urgent needs of patients.
